<p>This year Physiopedia is 10 years old (how did that happen?!). What started as a small playful project in 2009 is now the largest Physiotherapy and Physical Therapy resource in the world. With over 3000 evidence based articles, it is used by over <a href="https://www.physiospot.com/physiopedia/one-million-visits-thank-you/">1 million individuals every month</a> and reaches every country in the world!</p>
<p>The entire project is based on collaboration and everyone is included. Over the next few weeks, in the lead up to the <a href="https://www.physiospot.com/physiopedia/will-you-be-joining-physiopedia-at-wcpt-congress/">2019 WCPT Congress</a>, we will be sharing more information on the past, present and future collaborations and how you might choose get involved. Here’s a short snippet of what is to come:</p>
<ul>
<li><strong>Universities</strong> – Physiopedia <a href="https://www.physio-pedia.com/Content_Development_Projects">content development projects</a> started in 2009, these continue today plus Physiopedia and Physioplus are being integrated into teaching programmes as part of student education.</li>
<li><strong>Clinics</strong> – clinics world-wide are signing up to <a href="https://members.physio-pedia.com/why-physioplus">Physioplus</a> so that their staff have easy access to quality online continuing education and professional development tools, such as online courses, CEUs and a portfolio.</li>
<li><strong>Platforms</strong> – in a new development Physiopedia, Physioplus and Physiospot can now be added to platforms, such as EMR’s and LMS’s, to provide a free knowledge resource for users, enabling them to easily access knowledge at the point of care or learning.</li>
<li><strong>Member Organisations</strong> – similar to clinics, national member organisations are partnering to provide full access to the <a href="https://members.physio-pedia.com/why-physioplus">Physioplus continuing education and professional development tools</a> as a member benefit and recruitment/retention strategy.</li>
<li><strong>Topic Specialists</strong> – with specialist and expert knowledge are collaborating with Physioplus as a platform to <a href="https://members.physio-pedia.com/physioplus-course-presenter/">share their knowledge globally</a>. This knowledge translation platform is unique in that Physio/PT teachers can now reach every Physio/PT learner in the world through Physioplus.</li>
<li><strong>Clinicians</strong> – there are many ways that an individual clinician can contribute, culminating in the popular opportunity to <a href="https://www.physio-pedia.com/Volunteer">volunteer</a> on the Physiopedia editing team.</li>
<li><strong>Companies</strong> – and if nothing else, some c<a href="https://www.physio-pedia.com/Become_a_Physiopedia_Partner">onscious and socially responsible organisations</a> are simply making a donation to the Physiopedia charity so that we can continue to keep doing all that we are doing.</li>

<p><em>An open access article, published in the <a href="https://onlinelibrary.wiley.com/doi/full/10.1111/joa.12994">Journal of Anatomy</a> on the 17th of April, has caused a small sesamoid bone to hit headlines around the world – the Fabella.</em></p>
<p>If you haven’t heard of the fabella before we have an <a href="https://www.physio-pedia.com/Fabella_syndrome">excellent page</a> on Physiopedia which quickly bring you up to speed on all you need to know but here are a few facts:</p>
<ul>
<li>Fabella is Latin for little bean<img class="alignright" src="https://www.physio-pedia.com/images/thumb/9/91/Fabella_image.jpg/300px-Fabella_image.jpg" /></li>
<li>It is a small sesamoid bone located behind the lateral femoral condyle which is embedded within the lateral head of gastroc</li>
<li>Prevalence ranges from 3% to 87%</li>
<li>Highest rates reported in Asians and Australians</li>
<li>It’s exact function is not known but it is more common in non-human mammals</li>
<li>The fabella is known to complicate some knee replacements due to it ‘snapping’ over the replacement condyle which lacks a articulating groove</li>
</ul>
<p>The reason this study hit mainstream media is because this systematic review suggests that the prevalence of the fabella is increasing over time in the Korean population they investigated. It is currently unclear why this sesamoid bone is increasing in prevalence whereas others are not. The leading theory is because of humans increasing in height and weight we need the fabella to help increase the mechanical advantage of our calf muscles – in a similar way to all other sesamid bones.</p>

<p>Physiopedia strives to provide up to date resources and information to the Physiotherapy Community and to do this it relies on <a href="https://www.physio-pedia.com/Volunteers">volunteers</a> from around the world. Qualified physiotherapists/physical therapists and students alike work together to make this possible. The work and collaboration of these dedicated volunteers is appreciated and as a token of their efforts we have a badge system which represents the level and dedication of these unique and wonderful individuals, who share with us a wealth of experience and knowledge. Physiopedia is always evolving and moving forward, as the world around us changes, and one of the recent initiatives was to give the badges a make-over, and they look amazing. This was made possible by the hard work of Lauren one of our yellow belt volunteers. Badges are awarded depending upon the length of time our volunteers have been with us. So, what are our badges and what do they mean? Like the system used for Marital Arts each badge represents a belt and the colour reflects the achievements and progression of our volunteers, as each year passes. We have one volunteer who has achieved her black belt, but we are confident that over the next few years this number will grow.</p>
<ul>
<li><strong>White Belt</strong>− The birth of a new light is indicated by white. This is the first belt that is given to our volunteers at the start of their journey</li>
<li><strong>Yellow Belt</strong>− It symbolizes the first ray of sun light. In Physiopedia this badge is awarded to all Volunteers who pass the Volunteer course as they take on more responsibility and begin their journey as independent contributors.</li>
<li><strong>Orange Belt</strong>− This light is an indication of spreading of light on the earth. At this stage our volunteers have been working tirelessly for a year and have helped to expand the knowledge that Physiopedia shares with the Global Therapy Community.</li>
</ul>
<p>Each year our volunteers are with us they are awarded a new belt representing the hard work and dedication that they bring to the team, without these amazing individuals Physiopedia would not be the wonderful resource it is. Another belt is another step, on this rewarding and exciting journey, moving from Green to Blue to Purple, Red, Brown until the ultimate achievement the Black Belt. Just like in Martial Arts black is the highest accolade and symbolises enlightment and that every bright object casts a shadow (black). Volunteers who reach this level are not only strive to improve their skills but are also happy to share and use their knowledge to guide volunteers and continue to help Physiopedia grow. <p>Cardiopulmonary disease is an umbrella termed used to describe disorders of the heart (cardio) and lungs (pulmonary). Physiotherapy plays a big role in the rehabilitation and management of symptoms. Cardiopulmonary physiotherapists work with patients in a variety of settings and treat acute problems like asthma, acute chest infections and trauma chronic cardiac and respiratory conditions like Chronic Obstructive Pulmonary Disease (COPD), cystic fibrosis (CF) and post-myocardial infarction (MI).</p>
<p>The management of these conditions will vary and will be dependent on many factors but wherever possible conservative and non-invasive methods are favoured with emphasis on self-management. The role of the cardiopulmonary physiotherapist is to educate patients and also to be on hand when self-management is not enough. They have a range of skills and knowledge ranging from manual and breathing techniques to non-invasive ventilation and often use a combination of modalities to aid the clearance of mucus and ease the work of breathing. Physiotherapists can also plan exercise programmes to improve fitness, exercise tolerance and increase lung capacity.</p>
<p>Treatments will vary from client to client but there are guidelines, based on the best evidence available to assist clinicians to find the best the most effective interventions. The British Thoracic Society have recently updated their Clinical Guidelines on the Management of Patients with Bronchiectasis, these along with other guidelines can be found on the Physiopedia page <a href="https://www.physio-pedia.com/Clinical_Guidelines:_Cardiopulmonary">Cardiopulmonary Guidelines</a>. In fact, there is a whole category dedicated to <a href="https://www.physio-pedia.com/Category:Cardiopulmonary">Cardiopulmonary Disorders</a>.</p>

<p><em>We recently finished another round of the increasingly popular Physiopedia Volunteer Orientation Course. As part of the final assignment members were tasked to write an original piece of work to share with the profession, the contributions were of the highest quality. Below is the great piece of work written by <strong>Mandy Roscher</strong>.</em></p>
<p>Physiotherapists having been successfully helping women with pelvic floor dysfunction for a long time. Men’s Health Physiotherapy is now slowly gaining momentum. With more research being done every year we are realising that interventions focused on the pelvic floor muscles can help with a variety of pelvic problems in men. Cohen et al, 2015 published a literature review that introduces the readers to the basic functional anatomy of a male pelvic floor, its relationship to sexual function and how pelvic floor muscle rehabilitation can help.</p>
<h3>How Does an Erection Happen?</h3>
<p>An erection occurs due to a complex interaction between the vascular system, the parasympathetic, sympathetic, somatic and central nervous system with hormones and the muscular system also playing a significant role. An erection occurs when blood flows into the corpus cavernosa (tubes within the penis) faster than it can leave the penis and a closed hydraulic system maintains the erection.</p>
<h3>What Role Do The Pelvic Floor Muscles Have In Erectile Function?</h3>
<p>All the pelvic floor muscles (superficial and deep) have shown to be active during the period of an erection and orgasm. The ischiocavernosus helps to maintain the rigidity of an erect penis and fast contraction of the bulbospongiosus muscles are important for ejaculation.</p>
<h3>What Can Happen To The Muscles?</h3>
<p>The pelvic floor muscles can become weaker or hypoactive or they can have a higher resting tone which is known as a hyperactive pelvic floor. Both can affect erectile function indifferent ways. Pelvic floor muscles can become weaker with age and inactivity. They can also become hypoactive from nerve damage such as post prostatectomy. If they are under active they will not be able to help maintain an erection or allow for ejaculation and orgasm. Pelvic floor muscles can become hyperactive in men with Chronic Prostatitis and Chronic Pelvic Pain Syndrome, This results in a higher resting tone or “tight” pelvic floor. It is thought that the when the pelvic floor muscles have a resting tone that is too high it may compress the lumen from the outside which will limit the blood flow to the penis thereby affecting the erection.</p>
<h3>How Can Physiotherapy Help?</h3>
<p>There is more and more evidence coming out that pelvic floor muscle retraining can be very effective in treating erectile dysfunction. A detailed assessment by a suitably qualified physiotherapist will determine if the pelvic floor muscles are playing a role. The most important part is to determine if the pelvic floor is hypoactive or hyperactive. Giving pelvic floor exercises will significantly improve erectile function in someone with an underactive pelvic floor but will make a hyperactive pelvic floor much worse. Men with overactive pelvic floor muscles need to work on down-training or relaxing their pelvic floor muscles rather than strengthening.</p>
<h3>Conclusion</h3>
<p>While the ability to achieve and maintain an erection is multifactorial, pelvic floor musclesplay a vital role. Pelvic floor muscle training has been shown to be an effective therapy in the treatment of erectile dysfunction. They could even be the missing link for some men that are struggling and have tried other interventions.</p>

<p>Thanks to our recent collaborations with <a href="https://humanity-inclusion.org.uk/en/index?gclid=Cj0KCQiAvqDiBRDAARIsADWh5TfZh_8ELFmJ0iNwCdRwGSzBCG-yx6oYX1KvkjXlZ_hrEXb7vX1iuDsaAgdqEALw_wcB">Humanity Inclusion</a> we have been able to translate our Cerebral Palsy course into French! This is a massive boost to our French speaking colleagues in France, Africa and Haiti as well as our partners in Guyana, Niger, Senegal, Ivory Coast and Mali.</p>

<p>Cerebral Palsy is one of the commonest disorders of child development with an incidence of 2-2.5 per 1000 live births. <p>Some of the best work on Physiopedia has been created by physiotherapy students who have taken part in our <a href="http://www.physio-pedia.com/Educational_Projects">Educational Projects</a>. For students the projects offer an opportunity to create lasting resources for others on a global scale, for educators it offers an opportunity to involve their students in the knowledge creation process. This is a unique opportunity and students are often as excited about the projects as we are.</p>
<p>Put simply, students create resources in Physiopedia based on the content of their module or course content. This then demonstrates their understanding of a topic as well as contributing to this professional resource. The projects can provide opportunities for students to practice reviewing and disseminating literature related to current guidelines in a contemporary way. It is an interactive forum between educators and students allowing collaboration, and exposure to international learning opportunities. It really is a creative and unique way to develop academic skills such as reflection, critical assessment, writing and referencing skills.</p>
<h3>The Neuromotor Function Project</h3>
<p>A really excellent example of how the projects on PP come together to link <img class="alignright" src="https://www.physio-pedia.com/images/thumb/e/ed/Queens-university.jpg/260px-Queens-university.jpg" alt="Queens-university.jpg" />learning skills and knowledge creation is the <a href="https://www.physio-pedia.com/Queen%27s_University_Neuromotor_Function_Project">Neuromotor Function Project</a> which forms part of the “Neuromotor Function II” module in the first year of the MSc in Physical Therapy at Queens University. The students work in groups and create or update pages on PP that relate to neurological conditions which affect movement problems in adults.</p>
<p>Last year there were some top quality articles created including the topical subject of “<a href="https://www.physio-pedia.com/Modified_Constraint-Induced_Movement_Therapy_for_Stroke"><em>Modified Constraint-Induced Movement Therapy for Stroke</em></a>” and “<a href="https://www.physio-pedia.com/RTMS_Treatment_For_Parkinson%27s_Disease"><em>Repetitive Transcranial Magnetic Stimulation for the Management of Parkinson’s Disease</em></a>“. These pages are created with the utmost quality and care in keeping with the PP house rules. To ensure this quality there is a <a href="https://www.physio-pedia.com/Guide_To_Taking_Part_In_A_Student_Project">guide for students</a> too!</p>
<p>Clearly content creation is an important part of PP but the refresh and review aspect of the projects is arguably more important. Thanks to Queen’s University there are numerous pages which are updated to keep clinicans up-to-date with the latest evidence to give their patients the best treatment possible.</p>
<p>It can definitely be said that some of the best work on Physiopedia has been created by Physiotherapy Students who have taken part in our educational projects and Queen’s University are a shining example of that.</p>

Stretching a muscle with high tone works, AFTER the muscle fatigues or you stretch long enough to change the perception of stretch. That's not normally a rapid process as the more the nervous system perceives stretch, muscles tend to increase in tone to fight the passive stretch.<br />
<br />
Inhibiting a muscle by passively slacking it rapidly reduces tone, so you can start dynamically moving into the restricted range of motion and then start loading once the pattern is reinforced.<br />

<p><em>We recently finished another round of the increasingly popular Physiopedia Volunteer Orientation Course. As part of the final assignment members were tasked to write an original piece of work to share with the profession, the contributions were of the highest quality. Below is the great piece of work written by <strong>Nicole Sandhu</strong>.</em></p>
<h3>The Link Between Weight and Fertility</h3>
<p>A study conducted by Kiel et al. (2018), examined the effect of high-intensity interval training (HIT) on women undergoing assisted fertilization through a pilot randomized control trial. Eighteen woman with a body mass index of over 25.0 kg/m2 were split into two groups: the control group “usual care” (n=10) and the intervention group “HIT” (n=8). The first outcome measure was “ongoing pregnancy” and the second was “insulin sensitivity, reproductive hormones, oxygen uptake, and body composition.” The study concluded that HIT significantly improved insulin sensitivity, VO2 peak, and abdominal fat. Furthermore, fertility outcomes may increase by improving insulin sensitivity and the regulating the hypothalamic-pituitary-adrenal axis, central obesity, as well as, cardiovascular fitness [2,3].</p>
<h3>Implications for Clinical Practice</h3>
<p>A limitation of this study is the small sample size, as it is a pilot study. However, it has been well documented that weight has a strong correlation with the outcome of assisted fertilization [2,4]. Physiotherapists may be well positioned to assist patients with this aspect of their fertility journey. The first step for physiotherapists is to become educated on the topic and aid in educating the public on the importance of exercise and weight management in fertility. Physiotherapists can have a more direct role through creating and implementing an exercise program to suit the needs of their clients. Exercise is a relatively inexpensive addition to “usual care” in improving the outcomes of assisted fertilization. Additionally, exercise may aid in the mental health of clients during this challenging time. The ultimate goal is to aid individuals in reaching their fertility goals and it is important that as healthcare providers we are informative and proactive regarding the breadth of issues that could be affecting their assisted fertility outcomes.</p>

<p>In this observational study, the relationship between physical function and level of consciousness was evaluated at hospital discharge and subsequently how this was effected long-term mortality. Before we delve into the study itself there are limitations of the study we need to be aware of. The study took place in a single ICU in South Korea and only included 246 patients so caution needs to be applied when extrapolating these findings to other ICUs in different countries as the results may not be applicable.</p>
<p>As ICU and critical care management improves mortality rates are decreasing but long-term outcomes remain poor and there is a need to understand why before we can implement appropriate and effective changes to shit the balance of surviving and maintaining quality of life. In essence the researchers investigated whether simplified measurement of physical function and level of<br />
consciousness by nurses could predict long-term outcomes after discharge in ICU patients.</p>
<p>On discharge from the ICU the nurses were measuring a number of things;</p>
<ol>
<li>Levels of opiates – converted to Fentanyl equivalence</li>
<li>Levels of Benzodiazepines – recorded using Medazolam</li>
<li>Delirium – Using RASS and CAM</li>
<li>Level of function – 3 Groups;
<ol>
<li>Alert and walking</li>
<li>Alert and bed bound</li>
<li>Not alert and bed bound</li>
</ol>
</li>
</ol>
<p>These scores and categories were then used alongside 2 year mortality to see if any relationship was found. As you could predict survivors at two years were more likely to have been alert and walking when discharged from the ICU than those who were bedridden.</p>
<p>What does this mean clinically? Well at this stage large sweeping conclusions cannot be made as a result of this study but for physio’s working in a critical care setting it does help highlight the importance of early rehab. If anything rehab cannot start soon enough. As more studies like this take place there will be growing support for 7 day ICU services which have a balanced rehab and respiratory focus.</p>

Tightness isn't "short" muscles. Stretching just leads to fatigue may reduce power/performance. The feeling of tightness is much better treated with strengthening/loading strategies. <p>Today is World PD Awareness Day and although the EPDA has decided not to lead a global #UniteForParkinsons campaign in 2019, today is an opportunity for us all to refresh our knowledge. For Physiotherapy specific information you need to look no further than our quality pages on PD on Physiopedia.</p>
<p>It is a real shame the EPDA aren’t leading a campaign this year their stats for the previous couple of years were staggering and really did a lot for highlighting the needs to people with PD.</p>
<ul>
<li>In 2017, 9,000 people from 89 countries used the <a href="https://twitter.com/search?src=typd&q=%23UniteForParkinsons" target="_blank" rel="noopener">#UniteForParkinsons hashtag</a>, creating 33,000 social media engagements.</li>
<li>In 2018, the hashtag was used 17,000 times from 97 countries, our campaign video (see below) was seen 18,000 times, and we created a staggering 880,000 social media engagements.</li>

<p>The movement has now evolved into UNITED FOR PARKINSONS, information about the group is available <a href="https://www.unitedforparkinsons.com/">here</a>. The campaign ambitious conclude today so keep a close eye on their achievements in the coming days. Keep a particular look out for the second part of the campaign “This is Me”, which saw people living with PD from around the world share a photo and story which can be viewed online. diagnosed with Parkinson’s disease, across the globe, taking ownership of how they wish to be seen on the website. When viewers click on each picture, they will also be able to view the accompanying “hopes and dreams”.</p>
<h3>What is PD?</h3>
<p>Parkinson’s disease is a progressive neurological disorder affecting approximately 53 million people globally, making it the second most common behind Alzheimer’s dementia. The most common symptoms are bradykinesia, rigidity and tremor, often a person will also go on to develop a cognitive impairment or dementia. Symptoms are very different from person to person however on average symptoms usually progress over 15 years from independent to dependent on carers.</p>
<p>The symptoms of PD are caused by death of dopamine excreting cells in the substantia nigra within the basal ganglia. The cells die as a result of an abnormal accumulation of the protein alpha-synuclein which accumulates within neurones called Lewy bodies.</p>

About a month ago, I lifted by stubborn 80# Golden Retriever into the back of my wife's SUV and "felt" a rib pain. If you follow my work, you know I don't believe in subluxations, facet dysfunction etc. I tried most of my self treatments and assessments and while I could get it feeling good in most activities. Deep breath, trunk rotation left, and heavy lifts were still 4/10 discomfort.<br />
<br />
So I sought out Northern PT's resident manual therapist expert for his opinion and treatment. Thanks to Matt Breton for the quick assessment and treatment! Breathing and rotation are better! Up to me to keep it!<br />

Today, we are very excited to announce the release of an <strong>EduApp</strong> for <a href="https://members.physio-pedia.com/" rel="noopener" target="_blank">Physioplus</a>. Please note this is not an App for your phone or tablet! This is a “learning tool” which can be added to an institution’s Learning Management System (LMS), which is the college website where course materials and activities are hosted.</p>
<p>The Physioplus EduApp provides an extremely simple way for colleges and universities to add Single Sign On (SSO) access to the wealth of courses and resources on Physioplus within a course on their existing Learning Management Systems (LMS). So a student can, with one click in a course, go straight to a personal Physioplus account. Here they can take a course, view a resource, watch a technique video, participate in the forum and/or record learning reflections in the Physioplus portfolio. This will allow students to seamlessly support their studies and also enable educators to incorporate Physioplus based learning activities directly within their courses.</p>
<p>The Physioplus EduApp is LTI compliant and so is designed to work with most major LMSs (e.g. Canvas, Blackboard, Moodle etc).</p>

<p>One to Watch is the latest addition to the Physiospot repertoire. Once a fortnight we will point you in the direction of a recently published research study protocol which we think will be worthy of keeping an eye out for the results.</p>
<p>This month a protocol was published in Physical Therapy the APTA physio journal which will investigate the effect of blood flow restriction training (BFRT) on quad strength pre and post ACL surgery. Strength is only the primary measurement of the study with biomechanics, morphology and physiology of the muscles also being investigated. If you haven’t come across BFRT before here is a video from CSM 2018 explaining what it’s all about.</p>

<p>As explained in the interview above, the method is currently niche and has the possibility of becoming mainstream. BFRT is not yet a regular approach to rehab but it is beginning to be considered in a wide range of conditions and diseases. Knees are probably the one area where the technique is most widely used but there is yet to be conclusive evidence on its effects. That is why this study is worth keeping an eye out for as it could boost the reputation of the technique.</p>
<p>It is important to be aware the safety of the technique clearly this requires extra training and only certain pieces of equipment have recieved FDA approval. Be Safe! Bloodflow is stopped / reduced when this technique is used and there are a whole host of risks here.</p>

<p>GTN (Glyceral Trinitrate) is a drug you are likely to be more familiar with in the context of angina rather than tendinopathy however there is a topical ointment which as some evidence suggests may aid tendon healing. This concept has been around for some 20 years and there has just been a new systematic review published on the topic.</p>
<p>Don’t go rubbing topical GTN into your patients tendons just yet. There is still a lack of large scale well designed RCTs to provide conclusive evidence for the use of GTN for tendinopathy. However, based on this systematic review there is good evidence for the use in short- and medium-term for the treatment of tendinopathies which fail to respond to loading programmes.</p>
